The Birth to 3 Program is an early intervention program for infants and toddlers with developmental delays and disabilities and their families. The Program is available in every county in Wisconsin. In Dane County, two programs provide services. Bridges for Families (608-204-6242) Bridges@icc-wi.org serves families who live in the Madison Metropolitan School District. Birth to Three Connections (608-273-3232) Connections@ucpdane.org serves families who live in outlying areas of Dane County and in other school districts. Mission Statement *There are no wait lists for referrals, evaluations, or planning meetings to determine eligibility and to develop the Individualized Family Service Plan (IFSP). *An eligibility determination meeting will take place 45 days following the child's referral for evaluation. *When children are determined to be eligible for the Birth to 3 Program an IFSP will be designed by the early intervention team (which includes the family). *Services detailed on the IFSP are to begin within a reasonable amount of time following the plan meeting (e.g. 2-4 weeks).
