Elder Care of Dane County | |
|
Elder Care of Dane County is proud to celebrate 25 years of service in 2001. Founded in 1976 as the Older Adult Day Center, Elder Care has expanded over the years to meet the growing needs of older adults. Building on our roots in adult day services, we've taken innovative approaches to providing personal care services and supports right in the home as well as coordinated health and long-term care for older adults with chronic health problems. From the beginning, Elder Care has attracted the support of a range of public and private partners, from United Way to Dane County Human Services to Medicaid and Medicare programs to the nationally-recognized Robert Wood Johnson Foundation. More recently, we've been pleased to work closely with other organizations that share our vision of independence for frail older adults along with support for caregivers, such as the Elderly Services Network and the Alzheimer's Association. We receive funds from several sources, including Medicaid, Medicare, Dane County, other state and federal programs, United Way, participants, families, foundations, corporate donors, and individual contributors. Mission, Vision and Values Statement for Elder Care of Dane County: It is the Mission of Elder Care of Dane County to: Provide elderly persons with high quality community-based health and long-term support services that are cost effective, and meet the unique needs of each individual. It is our Vision that we will: Achieve leadership in providing community-based services for elderly persons.
