Home Health United - Hospice: Staff is dedicated to helping individuals live their last days with pain and symptoms managed, and surrounded by the individuals and things they love in the home setting of their choice.
Hospice supports families who care for patients experiencing cancer, end stage cardiac disease, pulmonary disease, and renal disease.
This service involves a team of caring professionals including physicians, nurses, social workers, home health aides, counselors and volunteers. This team also works with the patient's personal physician to ensure effective pain management treatment.
Our staff provides medical, psychological and spiritual support to terminally ill patients and respite to their families. We also care for patients at the St. Clare Hospice House in Baraboo.
Counties Served: Adams, Columbia, Dodge, Green Lake, Iowa, Juneau, Marquette, Richland, Sauk and Vernon.
