Home Health United - Medical Social Services: Our Social Work team provides services that will improve and maintain the social, emotional, functional, physical and mental health status of our patients and their families. Certified by the state of Wisconsin, the department consists of qualified health care professionals who hold advanced degrees in Social Work.
Following an initial psychosocial assessment, our medical social workers will offer an array of comprehensive services to patients by providing direction, education, information and advocacy.
