The following programs and services are offered through Northwest Dane Senior Services (NWDSS) Inc.

CASE MANAGEMENT
A case manager is available to assist area residents age 60+ with information and referrals for resources and services available locally and throughout Dane County. Home visits may be scheduled. Confidential services are available to any senior living in the townships or villages of Black Earth, Cross Plains, Mazomanie, Berry, Roxbury or Vermont. Information on financial benefits and supports; prescription and medical assistance; home chore and personal care assistance; housing options; caregiving tools; physical and mental health and wellness issues. NUTRITION SITE
Lunch is served at The Connection Monday-Friday at 11:30a.m. The site is closed for holidays and snow days determined by the Wisconsin Heights School District. To reserve a meal, please call The Connection by NOON of the previous day. If you call the day of to eat, it will be up to the staff, if you can attend. Dane County residents, age 60+, are asked to donate what they can afford. Guests under 60 are welcome at the full cost of the meal, $5.75. You are encouraged to give what you can afford. All first-timers dine for free. If you need to cancel, please contact our Nutrition Coordinator, Cindy at 767-3757 x 227 as soon as possible. HOME DELIVERED MEALS
Available for older adults that are unable to leave their home and meet the guidelines set by Dane County Nutrition Program. Meals meet 1/3 of the daily nutrition requirements for individuals age 60 and older. Meals are delivered Monday-Friday with an additional meal on Friday for use on Saturday. S.H.A.R.E. Orders
S.H.A.R.E. a nonprofit food buying club that offers good, fresh, nutritious products at a reduced cost through a volunteer run, community based distribution system. The order deadline with payment is June 9th. Stop by NWDSS if you need and order form. You may pick your order up on Saturday, June 27th. If you place your order on line, please notify us so we can be sure it is sent here for pick up.

SUPPORTIVE HOME CARE
Qualified staff provides services to our area older adults 60+, who require assistance with light housekeeping, meal preparation, laundry, lawn care and in-home respite care. Individual fees vary based on the participants' financial status and ability to pay.Please for more information call our Case Manager, Deanna at 767-3757 x 223, Or contact via email at case manager.(sliding fee)

RURAL RESPITE
A half-day program designed specifically for older adults who require structure in their day. Mondays in Mazomanie from 9:00am to 1:00pm. Please for more information call our Case Manager, Deanna at 767-3757 x 223, Or contact via email at case manager.(sliding fee)

DURABLE MEDICAL EQUIPMENT & SUPPLIES
Available through our LOAN CLOSET; a wide variety of equipment and supplies including wheelchairs, walkers, reachers, etc. We also accept donations of durable medical equipment to loan out. If you are in need of supplies, or would like to donate items, please call our Case Manager, Deanna at 767-3757 x 223, Or contact via email at case manager.(Donation)

LOAN LIBRARY
Several books are available at The Connection for check-out. Dane County Library Service delivers books on a monthly rotating basis available for check out.

FOOT CARE CLINICS
A nurse comes the 3rd Tuesday of the month in the morning and the 4th Wednesday of every other month in the afternoon for a complete foot exam. A fee of $20 is charged by the nurse. You will need an appointment. Please contact NWDSS to schedule your foot care appointment

MASSAGE THERAPY
A licensed massage therapist provides chair massages the 3rd Tuesday of the month. A fee of $10 is charged by the therapist. You will need an appointment. Please contact NWDSS to schedule your chair massage.

BLOOD PRESSURE SCREENING
Screening usually take place on the 2nd and 4th Monday of each month. There is no charge for this screening.

LOW VISION SUPPORT GROUP
Low Vision Support Group will take place on Thursday, June 18th over lunch at The Connection to offer support to one another, information and tips on living with Low Vision and share a laugh or two. If you have any helpful tips or new gadgets to share, feel free to bring them along. Call the office at 767-3757 to reserve a ride on the bus (it's only $1) and reserve your lunch (for those over 60, lunch is available by donation). BODY IN BALANCE
This class is full with 13 seniors. This class will be held at the BE State Bank, basement. In just one hour, one day a week, for eight weeks, seniors will learn some easy ways to enhance balance and mobility.

SENIOR WATER EXERCISE
New to the Mazo Pool summer program. Gentle Water Exercise for seniors, supervised by an exercise coach. This program will include fun, fitness and social activities. Gentle Water Exercise begins in June from 11:30-12:30. Please contact Kerri at (608) 239-0144 for more information.

RSVP Driver Escort
RSVP volunteer drivers give rides for medical and medically related appointments in Dane County on a donation basis to RSVP. Requests require a 3 day notice. Call NWDSS to set up a ride at 767-3757. A donation is requested to RSVP of Dane County per ride received.

CAREVAN BUS TRIP FARES
Service is available for all events of NWDSS e.g. Activity committee meetings and newsletter folding contact office for your bus reservations. All pick ups are at your homes unless otherwise announced. A minimum of 3 people must be met for trips into Madison and Baraboo. Any requests for last minute stops when the bus is out for a scheduled trip will be up to the discretion of the bus driver. * Rates: (All rates are one way) In town - $.50 cents (Nutrition, NWDSS Service Area) Shopping in area $1.00 (CP, BE, and Mt Horeb) Out of town $1.50 (Middleton, Madison, Sauk) No one will be denied service for inability to pay.
